Six questions to put to any hormone provider

Hormone therapy is now sold in many ways, some of them entirely online. Whoever you choose, these questions separate a medical program from a subscription. Here is how each is answered in our office.

1. Who is responsible for my prescription? Your plan is the responsibility of Dr. Donna D’Alessio, MD, who owns the practice; her board certification is in Physical Medicine & Rehabilitation. Nurse practitioner Tanya Santoro, NP-C, with more than two decades in nursing, brings a particular focus on testosterone therapy.
2. Will you test my levels before prescribing? Always. Your hormones are measured first, and the results are read next to how you actually feel.
3. Will anyone examine me? Yes. Your history is taken face to face at 624 Main Road, not through a web form.
4. How will my dose be adjusted? Through repeat labs and in-person reviews, with changes made on the evidence. The aim is a normal range in which your body works well.
5. How can I take it? As an injection, which allows precise and adjustable dosing, or as a cream or gel applied to the skin each day.
6. What if hormones are not my answer? Then you will be told so. A personal or family history of hormone-sensitive cancer, blood clots or certain heart and blood-vessel conditions can make therapy unsuitable, which is why the history comes before any prescription.

The first months on a plan

Energy, mood and sleep are often the earliest changes, typically within two to four weeks. Libido, body composition and mental sharpness usually follow over two to three months as levels settle into a steady range. While the body adjusts, some people have mild bloating, breast tenderness, a few breakouts, fluid retention or a reaction where an injection went in, and these generally ease once the dose has been fine-tuned.

Will hormone therapy make my weight worse? It does not directly cause weight gain, though a little water retention can come and go at the start. Many patients find that balanced levels make weight easier to manage, and weight-loss injections are offered in the same practice if more help is needed.

Will my insurance pay? That varies a great deal between plans. Lab testing and some prescription hormones are covered by certain plans, while compounded preparations often are not, so your insurer is the one to confirm. Our team can tell you what to expect to pay yourself.

Are there evening or weekend appointments? Tuesdays run until 7 in the evening, and Saturday appointments are available from 9 until 1.
