Weight care in a practice that also treats joints
Montville MedSpa & Pain Center is a pain center as well as a MedSpa. In the same office where weight-loss injections are prescribed, Dr. Donna D’Alessio, MD treats knees, backs and other joints without surgery, and the practice’s own guidance on joint pain includes keeping a healthy weight so the joints carry less stress. For some patients, then, the weight conversation and the joint conversation turn out to be connected. For many others the reason to come in is simpler: diet and exercise on their own have stopped moving the scale.
The two prescriptions
Semaglutide, the active ingredient in Ozempic® and Wegovy®, mimics GLP-1, a hormone the gut releases after eating that helps govern appetite and blood sugar. Tirzepatide, the ingredient in Mounjaro® and Zepbound®, works on GLP-1 and on the receptor for a second gut hormone, GIP. Each is a once-a-week injection under the skin, started low and raised in steps. Neither is prescribed until our medical professionals have reviewed your health, your goals and your medical history; a personal or family history of medullary thyroid carcinoma or MEN2, or a pregnancy, rules both out.
Keep a one-page log between visits
Regular check-ins are part of every plan, and they are most useful when you arrive with notes rather than impressions. A simple weekly record covers most of what your provider will ask about:
- Appetite — how quickly you feel full, and how often food is on your mind
- Side effects — especially in the few days after a dose goes up, when digestive symptoms tend to flare
- Water and protein — a smaller appetite often means drinking less, and protein at meals helps protect muscle
- Strength work — any resistance exercise you managed that week
- Sleep — short nights make hunger and cravings harder to manage
- Measurements — your weight, and a waist measurement if you like
From that, the dose, the eating advice and the exercise guidance can be adjusted to how you are actually doing.
Getting to the check-ins
The office sits about eight miles from West Caldwell, at 624 Main Road in Towaco. Route 46 does much of the work, and the whole drive takes about 16 minutes without traffic. On Tuesdays there are appointments until 7 pm, and Saturday mornings are open from 9 to 1.
West Caldwell patients ask
The scale barely moved in my first month. Is something wrong? Probably not. The starting dose is deliberately low, so progress in the early weeks is often modest and tends to build as the dose rises.
Apart from my stomach, what should I watch for? Report any change in your vision, particularly if you have type 2 diabetes, and any lump or swelling in the neck or new hoarseness. Low mood that is new or getting worse also deserves a call to your provider; in a crisis, call or text 988.
Can the injection do the work without changes to my diet? Not on its own. The medicine makes eating less easier to keep up, but your plan pairs it with dietary recommendations and exercise guidance because that combination is what the medication was studied with.

What Our Patients Say About Us
★★★★★
Shannon from the MedSpa is absolutely amazing! She took the time to really understand my skin and helped me figure out exactly what it needs. She recommended products that feel incredible on my skin and made the whole experience feel personalized. The room was so relaxing, and everything about the appointment was perfect. I’m someone who usually pushes off getting facials, but after finally getting one done, it was 100% worth it. Overall a 10/10 experience!
