One program, two sets of readers

Hormone replacement therapy at Montville MedSpa & Pain Center is for men and for women, and the two halves of the program start from different places, so this page takes them one at a time. Three rules hold for everybody: your levels are measured before anything is prescribed, every plan is overseen by Dr. Donna D’Alessio, MD, and you are seen face to face in Towaco rather than on a screen.

If you are a man

Men rarely walk in saying their testosterone is low. More often the gym has stopped paying off, the afternoons drag, desire has faded and patience runs short. Each of those can have more than one cause, so the first step is a blood test, and testosterone replacement therapy (TRT) is offered only if the result confirms a low level. The target is a healthy, working range, never the highest reading a laboratory can produce. Nurse practitioner Tanya Santoro, NP-C, who holds a master’s degree in adult health nursing from Fairleigh Dickinson University, brings a particular focus on testosterone therapy to the program.

If you are a woman

Shifting estrogen and progesterone seldom show up as one tidy symptom. Hot flashes and night sweats can arrive alongside poor sleep, foggy thinking, mood swings, a lower libido or weight collecting at the waist. Estrogen and progesterone therapy is offered through perimenopause, menopause and post-menopause, and your dose is set from your labs and your symptoms read side by side. In the first weeks some women notice bloating, tender breasts or a few breakouts while levels adjust; these generally fade as dosing is refined.

What is the same for both

• How you take it. An injection allows exact dosing that can be changed in small steps; a cream or gel goes on the skin each day.
• When you might notice it. Energy, mood and sleep often shift within two to four weeks, while libido, body composition and mental clarity tend to follow over two to three months.
• Who should not have it. A personal or family history of hormone-sensitive cancer, blood clots or certain heart and circulation problems can make therapy unsafe, which is why your history is taken first.
• How long it lasts. Some patients use it for one stage of life and later taper off, while others continue long term after reviewing it with their provider.

The old Horseneck road

Caldwell grew up in the middle of the Horseneck Tract, land bought from the Lenape in 1702, and Horseneck Road is still part of the drive to us. Bloomfield Avenue takes you west to Route 46, Hook Mountain Road and Horseneck Road link to Pine Brook Road, and that carries you north into Towaco and on to 624 Main Road. Allow close to 18 minutes without traffic for roughly eight and a half miles.
