When your body changes and your habits have not
One of the commonest reasons people ask about hormones is a body that has started to behave differently: weight gathering at the middle, muscle tone slipping in spite of the gym, energy gone by mid-afternoon. Hormone replacement therapy at Montville MedSpa & Pain Center is offered to men and to women for exactly this kind of change. It helps to be clear about what it can and cannot do before you book.
For men, testosterone
Low testosterone can bring fatigue, less muscle, a lower libido, irritability and brain fog, and men often mention belly fat that refuses to shift. Testosterone replacement therapy (TRT) is offered only after lab testing confirms that the level really is low, and the dose is then set to bring it back into a healthy range for stamina, strength and focus.
For women, estrogen and progesterone
Through perimenopause, menopause and the years after, changing estrogen and progesterone can show up as hot flashes, broken sleep, mood swings, a fading libido and weight that settles around the waist. Estrogen and progesterone therapy is planned from your own lab results read beside your symptoms.
Hormones and weight, honestly
- HRT is not a weight-loss medicine. Nor does it directly cause weight gain; any early puffiness is usually water retention, and it tends to pass.
- It can make the basics easier. Many patients find that hormone levels brought into balance support better weight management, by way of their metabolism, energy and muscle tone.
- Shape changes slowly. Energy, mood and sleep often move within two to four weeks, while body composition, libido and mental clarity usually take two to three months as levels settle.
- If appetite is the real obstacle, weight-loss injections, semaglutide or tirzepatide, are prescribed within the same practice after a consultation of their own.
How the decision is made
Every plan starts with a consultation in the Towaco office and lab work, and nothing is prescribed until your levels have been measured. A personal or family history of hormone-sensitive cancer, blood clots or certain heart and circulation problems can make therapy unsuitable, so those questions come early. Treatment is given by injection or as a cream or gel applied to the skin, and follow-up labs keep the dose in step with your results. Dr. Donna D’Alessio, MD, board certified in Physical Medicine & Rehabilitation, oversees every plan, and nurse practitioner Tanya Santoro, NP-C has a particular focus on testosterone therapy.
Cedar Grove patients ask
Will my insurance pay? Coverage varies widely. Some plans pay for lab testing and certain prescription hormones, while compounded preparations are often not covered. Your insurer can confirm, and our team can explain what may be billable.
How far is it? About 16 minutes without traffic: Route 23 north to Mountainview Boulevard, then Route 202 through Lincoln Park to 624 Main Road in Towaco.
Can I check my symptoms before booking? The 30-second hormone quiz is a quick way to see whether a consultation makes sense.
Are there late or weekend hours? Tuesdays until 7 pm, and Saturday mornings from 9 to 1.

I have been to see Dr. D'Alessio over the past 5 years. I especially drive for over an hour, she and her whole office are simply the BEST! She has done my leg veins, Botox and now I see her for my GLP weight loss treatments, she is truly AMAZING and CARING! She will work with your insurance and help you find the best course of treatment to bring the cost down, if possible. She is always conservative, attentive, kind and has so much patience when listening to your exact needs. I would never go anywhere else. I trust her and her staff fully!
