Where the lower face changes

Some of the most noticeable changes that come with age sit below the cheekbones: the folds running from the nose to the corners of the mouth, the lines dropping from the mouth toward the chin, and a jawline that has lost its edge. Much of that comes from volume, the fat and collagen that thin over the years, and replacing volume is what hyaluronic acid filler does best. Working from the top down

• Begin above the line. The midface is one of the earliest areas to lose volume, and as it flattens, the folds beside the mouth look deeper. Restoring the cheekbones with Juvéderm Voluma XC has a subtle lifting effect on the tissue below, which can soften those folds as well.
• The nasolabial folds. These smile lines, from the nostrils to the corners of the mouth, are often treated with Juvéderm Ultra Plus XC, placed deep, or with Vollure XC, chosen for longer-lasting smoothness.
• The marionette lines. The vertical creases below the corners of the mouth can be lifted and smoothed the same way, without stopping your face from moving naturally.
• The chin and jawline. Juvéderm Volux XC is a structural gel that sharpens definition along the jaw and can bring a receding chin forward, for a firmer profile with no surgery involved.
• Around a mouth that never rests. RHA is pliable enough for areas in constant motion, and Revanesse Versa was the first HA filler FDA-approved for dynamic wrinkles and folds.

What filler will not do down here

Fine lines around the lips and a dimpled, pebbly chin can come from muscle rather than lost volume, and Botox or Dysport is the tool for those. Where skin along the jaw has loosened rather than hollowed, PDO threads add a lift that filler cannot, and the two can be combined. Your provider will tell you which applies to your face.

How the appointment runs

Injection points are marked by facial mapping, a numbing cream goes on 15 to 30 minutes beforehand, and most modern fillers also carry lidocaine. Once placed, the gel is shaped by hand and checked while you smile and frown, so the result keeps pace with your expressions. Treatment is led by Dr. Donna D’Alessio, MD, who is trained in facial anatomy and vascular safety, working with nurse practitioners Amy Natalicchio and Tanya Santoro, and you keep the same provider from consultation to follow-up.

The drive from Roseland

Allow about 20 minutes without traffic for roughly 11 miles. I-280 west carries you out of Roseland to the exit for Edwards Road and New Road; from there, New Road and a brief stretch of Bloomfield Avenue lead to Changebridge Road, which heads north to Main Road. How long does lower-face filler last? Most hyaluronic acid fillers last 6 to 18 months, depending on the product, the area and your metabolism, and the chin and jawline tend to hold filler longer than the lips do.

Can it be reversed? Yes. An enzyme called hyaluronidase breaks down HA gel if a correction is ever needed.

What should I avoid afterwards? For three to six hours, keep your hands away from the area. Give hard workouts 24 to 48 hours, and keep alcohol, blood-thinning medicines and strong sun out of the first day.
