What people expect, and what actually happens

Botox has been talked about for so long that most people reach a consultation already holding firm ideas about it, and several of those ideas are wrong. Here are six we hear, set against how Botox and Dysport work at Montville MedSpa & Pain Center.

Six assumptions, checked

“It fills in wrinkles.” Nothing is filled. Botox and Dysport block acetylcholine, the messenger a nerve releases to make a muscle contract, so the targeted muscle rests and the skin above it smooths. That is why they suit lines made by movement. A crease that remains while your face is completely still usually reflects lost volume, which is a job for dermal fillers.

“I will look frozen.” The practice’s approach keeps your face moving. On the forehead, for example, the dose is placed so the lines soften while your brows can still lift, and the result patients describe is relaxed rather than rigid.

“It works the same afternoon.” Softening usually begins somewhere between day three and day five, and the full effect takes 10 to 14 days to arrive. Judge it at two weeks rather than two days, and book well ahead of any occasion you want to look rested for.

“It is new and untested.” Botox received FDA approval for cosmetic use in 2002, having first been developed for eyelid spasms and excessive sweating. Dysport belongs to the same botulinum toxin type A family and works the same way, though the two are formulated differently and spread differently once placed.

“Once I start, I am locked in.” None of it is permanent. Over the following months your body metabolizes the product and the muscle gradually works again. Most people who carry on return every three to four months, with at least three months between touch-ups; anyone who stops simply finds movement coming back.

“It is purely cosmetic.” Neurotoxins are used medically too, for instance to ease the symptoms of an overactive bladder. In this practice, suitable candidates can be assessed for TMJ and jaw tension, chronic migraines and excessive underarm sweating, each with its own consultation.

Who treats you

Injections are given by Dr. Donna D’Alessio, MD, the practice’s founder, or by nurse practitioners Amy Natalicchio and Tanya Santoro. Dr. D’Alessio reviews and oversees every neurotoxin treatment, and her board certification is in Physical Medicine & Rehabilitation, a field concerned with how muscles and nerves behave. Placement is decided by your facial structure and the strength of each muscle.

Getting here from Roseland

Interstate 280 runs straight through Roseland, and it covers the first stretch. Take it west, signed for Parsippany, and leave at the exit for Edwards Road and New Road. New Road and a short piece of Bloomfield Avenue bring you to Changebridge Road, which runs north to Main Road; turn right, and our office at 624 is about a mile along. In all it is roughly 11 miles, about 20 minutes without traffic.

Roseland patients ask

Does it hurt? Each injection feels like a brief pinch, the needle is very fine, and numbing is rarely needed.

What should I avoid beforehand? Alcohol and blood thinners for up to a week, where that is safe to do. If a doctor prescribed your blood thinner, check with them before stopping anything.

Can I come after work? On Tuesdays we see patients until 7 pm, and Saturday mornings run from 9 to 1.
