The same program for men and for women
At Montville MedSpa & Pain Center, hormone replacement therapy is offered to men and to women. Men are treated with testosterone replacement therapy (TRT), and only after blood work shows their testosterone is low. Women are treated with estrogen and progesterone therapy, whether they are in perimenopause, menopause or the years beyond. This page is about what comes after the first prescription, because that is the stretch most people know least about.
Before the first dose
Nothing begins until you have had an office consultation, a full health history and lab work. A personal or family history of hormone-sensitive cancer, blood clots or certain cardiovascular conditions can rule therapy out, which is why those questions are asked early. You then choose how to take it: an injection, which allows precise dosing that is simple to adjust, or a cream or gel applied to the skin every day.
Weeks one to four
When the first changes come, they are usually in energy, mood and sleep, often within two to four weeks. This is also the window in which adjustment side effects tend to appear, if they appear at all: mild bloating, breast tenderness or mood swings while estrogen levels settle in women, a few breakouts, some fluid retention, or a reaction where an injection went in.
Months two and three
Libido, body composition and mental clarity usually take longer, around two to three months, as levels reach a stable range. Follow-up labs and an in-person review are how the dose is checked against both your numbers and your symptoms. The aim is a normal, functional range, never the highest figure a lab can print.
What to write down between visits
A review goes further when you arrive with notes instead of a general impression. A few lines a week is enough:
- Sleep — how long, how deep, and for women, how often night sweats woke you
- Energy — whether the afternoon slump is still there
- Mood — irritability, anxiety or a flat stretch
- Libido — any change, in either direction
- Skin and body — breakouts, bloating, swelling or tenderness
- The dose itself — redness at an injection site, or days the cream was missed
Notes like these help your provider decide whether to hold the dose, adjust it or change the protocol.
Who reviews your plan
Dr. Donna D’Alessio, MD oversees every plan; she owns the practice and holds board certification in Physical Medicine & Rehabilitation. Nurse practitioner Tanya Santoro, NP-C brings particular expertise in testosterone therapy. You meet both of them in the office, not through a screen.
From Roseland
Our office is about 11 miles from the middle of Roseland, roughly 20 minutes without traffic. I-280 west and Changebridge Road do most of the work, and the final mile runs east along Main Road to number 624.
Roseland patients ask
Do I stay on it forever? Not necessarily. Some people use it through one stage of life and later taper off, while others carry on for the long term. Your plan is reviewed regularly, and the choice remains yours.
Can I check my symptoms before booking? A good first step is the 30-second hormone quiz on our HRT page.
Are there late appointments? Tuesday appointments run until 7 in the evening, and Saturdays until 1.
