Tired all afternoon, awake half the night
Running out of energy and sleeping badly are two complaints that often lead people to wonder about their hormones. Hormone replacement therapy at Montville MedSpa & Pain Center treats both men and women. Men whose testosterone is confirmed low by blood work are offered testosterone replacement therapy (TRT); women are offered estrogen and progesterone therapy through perimenopause, menopause and the years beyond it. The way those two complaints show up differs between the sexes, so it helps to look at each.
How it tends to look
In women, sleep is often broken by hot flashes and night sweats, or simply never feels restful, and the tiredness that follows comes with brain fog, a shorter fuse and weight settling at the middle.
In men, the pattern more often starts in the daytime: stamina that fades well before evening, workouts that take longer to recover from, less drive, and irritability that does not feel like them.
In either, concentration slips, mood dips and intimacy loses its pull.
Why your blood work comes before any answer
Fatigue and poor sleep have plenty of causes besides hormones, so nothing is prescribed from a list of symptoms. At a consultation in our Towaco office you describe what has changed and give a full history, and your levels are then measured. Those numbers are read next to how you actually feel, since two people with identical results may need different things. A personal or family history of hormone-sensitive cancer, blood clots or certain heart and circulation problems can rule therapy out. If hormones are not behind your tiredness, you will be told that directly.
If treatment suits you
You choose between an injection, which allows exact doses that are easy to fine-tune, and a cream or gel applied to the skin each day. In the practice’s own experience energy, mood and sleep are often first to shift, commonly within two to four weeks, while libido, body composition and mental clarity tend to need two to three months. Results vary, and your physician sets expectations from your own labs. Repeat testing and in-person reviews follow, with the dose moved on evidence.
The physician answerable for every plan is Dr. Donna D’Alessio, MD, founder and owner of the practice and board certified in Physical Medicine & Rehabilitation. Nurse practitioner Tanya Santoro, NP-C brings a particular focus on testosterone therapy.
The drive from Cedar Knolls
Ridgedale Avenue carries you north to Route 10, and a few hundred yards later a ramp puts you on I-287 north for the longest stretch. Leave at Exit 47, turn left onto Main Road, and our office at 624 is about a mile and a half along: roughly eleven miles and 17 minutes without traffic by OpenStreetMap’s routing.
Hanover patients ask
Is this only for menopause? No. Half of the program is for men with lab-confirmed low testosterone, under the same lab-first rules.
Will I be seen by video? Never. Consultations and follow-up reviews take place face to face in Towaco.
Is there a quick way to check my symptoms? The 30-second hormone quiz on our HRT page is a quick first look.

What Our Patients Say About Us
★★★★★
We drive over 50+ miles to come see Kristen, particularly. I can’t say enough good things about her, the results, and the experience she makes when we come into this spa. Thank you for always going the extra mile for my two daughters and making us feel like we are at home away from home after having such a long drive. Definitely worth the money and the trip! The staff is also always very friendly, allowing me to switch appointments when I need to, and they are always so kind when I call. I am a picky customer, it is hard to make me happy. Definitely recommend this MedSpa!
