Below the eyes, a different set of muscles

Ask most people where a wrinkle relaxer goes and they touch their forehead. That is where treatment usually starts at Montville MedSpa & Pain Center, yet the practice’s list of neurotoxin areas carries on lower down the face. Four of them surprise people who have only ever thought about frown lines, and each one is made by a muscle, which is the only kind of crease Botox or Dysport can soften.

Four lower-face areas, one by one

• Bunny lines. The small creases that bunch across the bridge and the sides of the nose.
• Lip lines. The fine lines that collect around the mouth, drawn in by the ring of muscle that circles the lips.
• Chin dimpling. The pebbled, orange-peel surface some chins take on.
• Neck bands. The vertical cords at the front of the neck, known medically as platysmal bands.

Lower down, the mechanism is no different from the brow. Both products are FDA-approved botulinum toxin type A, and both keep a nerve’s chemical messenger, acetylcholine, from reaching the treated muscle, so it rests and the skin above it stops folding so sharply. Whether any of these four suits you is settled at a consultation, because the practice treats them for appropriate candidates rather than as a menu.

When the lower face needs another tool

Plenty of what changes below the cheekbones has nothing to do with muscle. Creases running from the nose to the corners of the mouth, and the marionette lines below them, usually come from lost volume, and the practice treats them with hyaluronic acid dermal fillers such as Juvéderm Ultra Plus XC or Vollure XC. A jawline that has lost its edge is a job for Juvéderm Volux XC, and skin that has loosened along the jaw and neck may be better lifted with a PDO thread lift. Your injector will say which applies before anything is drawn up.

Who places it

The syringe is held by Dr. Donna D’Alessio, MD, founder of the practice, or by nurse practitioner Amy Natalicchio or Tanya Santoro. Every neurotoxin plan, whoever gives the injections, is reviewed by Dr. D’Alessio, whose board certification is in Physical Medicine & Rehabilitation. Placement follows your own facial structure and the strength of each muscle, which matters around the mouth, where small muscles sit close together.

Whippany or Cedar Knolls to Towaco

From Whippany, follow Route 10 a short way west, turn north on Parsippany Road and pick up I-287 north. Exit 47 brings you to Route 202; turn left and Main Road leads about a mile and a half to number 624. OpenStreetMap’s routing puts it at roughly ten miles and 15 minutes without traffic, or nearer 17 minutes from Cedar Knolls by Ridgedale Avenue.

How long does a lower-face treatment last? For most people around three to four months, varying with the area, the product and your metabolism, and leaving at least three months between touch-ups is the usual advice.

What might I notice afterwards? A little redness, swelling or a small bruise at an injection point, generally gone within hours to a few days. Keep your hands off the treated area for 24 hours.
