Before you start hormones anywhere
Hormone replacement therapy is easy to find now, from local practices to websites that put a box in the mail. Whether a program suits you depends less on its name than on how it is run. These are the questions we would ask in your place, answered as they apply at Montville MedSpa & Pain Center, where men are treated with testosterone replacement therapy (TRT) and women with estrogen and progesterone therapy.
Six questions worth asking
1. Will I actually be seen? Here, in person. Consultations and follow-up reviews take place in our Towaco office, and your full health history is taken in the room rather than through a web form.
2. Who oversees the prescription? Every plan is overseen by Dr. Donna D’Alessio, MD, the physician who owns the practice, who is board certified in Physical Medicine & Rehabilitation. Nurse practitioner Tanya Santoro, NP-C has more than 20 years in nursing, over ten of them as an NP, and specializes in hormone therapy with a particular focus on testosterone. Hormones affect the heart and blood as well as mood, and Tanya’s background takes in cardiology and emergency medicine alongside primary care.
3. Are labs done before anything is prescribed? Always. Your hormones are measured first and the results are read together with your symptoms. For men, testosterone is offered only when testing confirms that it is low.
4. What could rule me out? A careful program asks before it prescribes. A personal or family history of hormone-sensitive cancer, of blood clots or of certain cardiovascular conditions can make therapy unsafe, so those questions come at the very start.
5. How will my dose be adjusted? Through repeat labs and in-person reviews. The target is a normal range where you function well, and the dose moves when your numbers and your symptoms say it should. Injections allow small, precise changes, while a cream or gel suits people who prefer a daily routine.
6. What if I want to stop? You can. Some patients use therapy through one phase of life, such as menopause, and then taper off; others continue for years. The plan is reviewed regularly, and whether to carry on remains your call.
The drive from North Caldwell
Greenbrook Road and Passaic Avenue lead you out toward Two Bridges Road, and Pine Brook Road and Chapel Hill Road take you the rest of the way to Route 202 in Lincoln Park. From there, 202 heads west into Towaco and past our office at 624 Main Road. Allow about 16 minutes without traffic for a trip of roughly eight miles.
North Caldwell patients ask
Is this mostly a women’s program? No. Testosterone therapy for men and menopause care for women are both part of it, with the same lab-first approach for each.
Are there side effects at the start? Some people have mild bloating, tender breasts, mood swings, acne, fluid retention or a sore injection site at first; these usually settle once the dose is right.
How is the cost worked out? It depends on whether you choose injections or a topical, on the dose and on how often you are treated, so the figure is set out at your consultation along with payment options.
When can I come in? We see patients six days a week, staying until 7 pm on Tuesdays and closing at 1 pm on Saturdays.
