Three reasons the scale stays put
Weight that will not budge is often more than one problem, and the fix depends on which one you have. At Montville MedSpa & Pain Center three situations come up again and again, and each has its own answer in the same office:
- Hunger that never seems to switch off. Appetite runs on gut hormones, and after a round of dieting the body tends to turn hunger up and resist further loss. This is the problem semaglutide and tirzepatide are built for.
- A shift in your hormones. Weight that has started collecting around the middle near menopause, or muscle tone slipping away in a man whose testosterone is low, may point somewhere else entirely.
- One stubborn area on an otherwise stable body. A pinchable pocket that ignores the gym is a contouring question, not a weight-loss one.
Your consultation sorts out which of these applies to you, and it is common to find more than one.
When appetite is the problem
Semaglutide imitates GLP-1, a hormone your gut releases after a meal: it acts on the parts of the brain that drive hunger, slows how quickly food leaves the stomach and helps insulin respond when blood sugar rises. Tirzepatide works on GLP-1 and on a second receptor, GIP. Either is a single injection under the skin once a week, in the stomach, thigh or upper arm, starting at a low dose that climbs in steps. Before anything is prescribed, our medical professionals go through your health, goals and history; a personal or family history of medullary thyroid carcinoma or MEN2 rules both medicines out, and so does pregnancy. Every plan pairs the medication with dietary recommendations, exercise guidance and regular check-ins.
When hormones are part of it
Hormone replacement therapy is for men and women, and it is not a weight-loss drug. It restores testosterone in men, or estrogen and progesterone in women, only after lab work shows a need. HRT does not directly cause weight gain, and many patients find that balanced levels make weight easier to manage.
When it is one stubborn area
CoolSculpting freezes fat cells in a targeted area such as the flanks, abdomen or under the chin. It suits people already near their ideal weight, never those with a great deal to lose. Where significant weight loss has left skin looser than you would like, body contouring can help with that too.
Side effects, briefly
The injections mostly cause digestive effects such as nausea, constipation or diarrhea, strongest in the days after a dose goes up. Tell your provider instead of skipping doses, and call promptly about severe stomach pain that will not settle or any swelling of the face or throat.
Essex Fells patients ask
How long is the drive for check-ins? About 21 minutes without traffic: up Roseland Avenue to Bloomfield Avenue and Route 46, then Pine Brook Road north to Main Road in Towaco.
Could I need more than one of these? Some people do. Which treatments make sense, and in what order, is decided with your provider.
Which days can I come? Monday through Saturday; Tuesday runs until 7 pm and Saturday until 1 pm.
Compare the two medicines in semaglutide vs. tirzepatide, or see the weight-loss injections page.
