The meals that do not get smaller
Starting semaglutide or tirzepatide changes how much you want to eat, but it does nothing to your calendar. Sunday dinners, birthdays, weddings and restaurant nights with friends keep arriving, and for many people they are the trickiest part of the early months. A little planning makes them easier, and none of it means staying home.
How the medicine changes a plate
Both medicines are one injection under the skin each week. Semaglutide imitates GLP-1, a hormone the gut releases after eating, and tirzepatide acts on GLP-1 together with a second receptor, GIP. Each slows the speed at which your stomach empties and quiets the signals that make you hungry, so you feel full sooner and stay full longer. That is how they help, and it is also why a portion that used to seem ordinary can suddenly feel like too much.
Before and during the big meal
- Choose a smaller plate, or ask for one. Going back for a second helping is easier than stopping halfway through a heaped first one.
- Make room for protein. With less space to fill, put meat, fish, eggs or beans on the plate before the bread basket gets a look in; protein helps protect muscle while weight comes off.
- Stop at the first sign of fullness. Pushing past it is the most common trigger for nausea.
- Pick carefully from the richest dishes. Fried, creamy and very sugary food tends to be the hardest to tolerate.
- Go lightly on wine and cocktails. Alcohol can make nausea and a poor appetite worse.
- Stay upright after eating rather than heading straight for the couch, which helps with both nausea and heartburn.
The check-ins in between
Regular check-ins are built into every plan here, after a consultation in which our medical professionals review your health, goals and history. They are where your provider hears how meals are going and how well you are tolerating the medication, and decides whether the dose or the eating advice should change. Side effects often come as a small wave for a day or two after an injection, most of all after a dose increase, so mention any big occasion coming up. Never skip or double a dose, or change your schedule, on your own; ask first.
From Totowa the office is about eight and a half miles away: Route 46 and Route 23 to Mountainview Boulevard, then Route 202 through Lincoln Park to 624 Main Road in Towaco, roughly 15 minutes without traffic.
Totowa patients ask
Can anyone take these medicines? No. Both are ruled out by a personal or family history of medullary thyroid carcinoma or MEN2 and by pregnancy, and your provider will also ask about pancreatitis, gallbladder and kidney trouble and every medicine you take.
What if I overdo it at a party? Expect to feel uncomfortably full, possibly queasy, for a while. Sipping water, staying upright and keeping the next meal plain often help. Severe stomach pain that will not go away, or being unable to keep fluids down, is a reason to call your provider promptly.
What about the holidays? Keep your injection on its usual day unless your provider says otherwise, and once the celebrations are over, go back to the everyday routine of protein, water, some strength work and your next check-in.
Are you open on Saturdays? Yes, from 9 am until 1 pm, and on Tuesdays appointments run until 7 pm.

Testimonials
What Our Patients Say About Us
★★★★★
My 17 year old daughter sees Dr. D’Alessio for hyperhidrosis (excessive underarm sweat) which for a teenager is emotionally embarrassing. I knew my daughter needed a provider who was able to be sympathetic to her needs and have a caring touch. Dr. D took the time to speak with my daughter and provide a treatment plan just for her. For her initial Botox treatment my daughter was very nervous. Dr. D was, as always, very patient with her and made her experience pain free! We wouldn't consider going anywhere else!
