Men and women, the same careful start
Hormone replacement therapy at Montville MedSpa & Pain Center is a program for both sexes. Men whose testosterone is confirmed low by lab testing are offered testosterone replacement therapy (TRT). Women are offered estrogen and progesterone therapy in perimenopause, through menopause and in the years beyond. Whichever applies to you, the first visit in our Towaco office works through three questions before anyone writes a prescription.
Question one: are hormones really the cause?
The complaints overlap more than people expect. Tiredness that rest does not fix, nights of broken sleep, foggy concentration, a fading sex drive and a waistline that will not shift can come from hormones or from somewhere else entirely. Women often add hot flashes and night sweats; men describe crashing by mid-afternoon and losing muscle in spite of the gym. Your levels are therefore measured, and the results are read next to how you actually feel, since an identical number can call for different decisions in two different people. When hormones turn out not to be the explanation, you hear that directly.
Question two: is it safe for you?
Hormones affect far more than mood and energy, so a full history is taken face to face. Hormone-sensitive cancers, blood clots and certain heart and circulation conditions, whether in your own past or your family’s, can put therapy off limits. Those questions are asked early, before any talk of doses.
Question three: how would you take it, and at what cost?
- By injection, which allows exact doses that are easy to change in small steps.
- As a cream or gel, put on the skin once a day, which suits people who like a daily habit.
- The cost follows from that choice, from the dose and from how often you are treated, and your provider sets it out in full at the consultation, along with payment options. Insurance varies widely: some plans pay for lab testing and certain prescription hormones, while compounded preparations are often excluded.
Once you have started
Follow-up labs and in-person reviews keep the dose matched to your numbers and your symptoms, aiming for a normal range in which you function well. Energy, mood and sleep tend to respond first, often within two to four weeks; libido, body composition and mental clarity usually take two to three months. Some people stay on therapy for years, and others use it to get through menopause and then taper off.
Every plan is overseen by Dr. Donna D’Alessio, MD, who owns the practice and is board certified in Physical Medicine & Rehabilitation. Nurse practitioner Tanya Santoro, NP-C, who earned her master’s in adult health nursing at Fairleigh Dickinson University, focuses particularly on testosterone therapy.
Coming back from Bloomingdale
Hormone care means repeat visits, and from Bloomingdale each one is about nine and a half miles: Main Street and the Paterson-Hamburg Turnpike to I-287 south, Exit 47, then Main Road to number 624. Allow close to 14 minutes without traffic.
Bloomingdale patients ask
What if my levels come back normal? Then hormone therapy may not be your answer, and you will be told so. The numbers are always judged alongside your symptoms rather than on their own.
Can my partner and I both be treated? Yes. Each of you has a separate history, separate labs and a separate plan.
When can I come in? Monday to Saturday, with Tuesday appointments until 7 pm and Saturday ones until 1 pm.

I have been to see Dr. D'Alessio over the past 5 years. I especially drive for over an hour, she and her whole office are simply the BEST! She has done my leg veins, Botox and now I see her for my GLP weight loss treatments, she is truly AMAZING and CARING! She will work with your insurance and help you find the best course of treatment to bring the cost down, if possible. She is always conservative, attentive, kind and has so much patience when listening to your exact needs. I would never go anywhere else. I trust her and her staff fully!
