Two kinds of first appointment
Weight-loss medication is easy to come by online now, sometimes after nothing more than a questionnaire. So the people who book a consultation at Montville MedSpa & Pain Center tend to fall into two groups: those who have never taken semaglutide or tirzepatide, and those who already take one and would like a medical team they can actually sit down with. The visit starts the same way for both, with our medical professionals reviewing your health, your goals and your medical history, but the questions that follow are a little different.
If you have never taken one
Semaglutide copies GLP-1, a hormone the gut releases after eating that tells the brain you have had enough, keeps food in the stomach for longer and helps insulin respond to a meal. Tirzepatide acts on GLP-1 and also on GIP, a second gut hormone involved in insulin release and energy balance. Both are given once a week under the skin. Injections generally suit adults whose BMI indicates overweight or obesity and who have struggled to lose weight through diet and exercise. If one suits you, the plan sets out the medication and dose, dietary recommendations, exercise guidance and a schedule of check-ins, and the dose begins low and rises in stages.
If you are already on one
- Bring the details: which medicine you take, the dose you are on, when you started, and how your stomach has coped.
- Bring the rest of the list: every other medication and supplement, plus any recent blood work.
- Expect the screening anyway. A personal or family history of medullary thyroid carcinoma or MEN2 rules both medicines out, as does pregnancy, and you will be asked about pancreatitis, gallbladder disease, kidney problems and diabetic eye disease.
- Do not change anything on your own in the meantime. Skipping, doubling up or switching should wait for your provider.
Moving from one medicine to the other is sometimes considered, for instance when one is hard to tolerate or not working well. It is done only under your provider’s guidance, because they decide the starting dose and the timing.
Two things that catch people out
If you take birth control pills, say so: tirzepatide can make them less effective after you start it and after each dose increase. And if you also use insulin or another medicine that lowers blood sugar, learn the warning signs of a low, which include shakiness, sweating, confusion and a racing heartbeat.
Getting to your check-ins
From the middle of Denville, Route 46 east carries you into Parsippany, where Littleton Road leads onto I-287 north. Exit 47 puts you on Route 202; turn left, and Main Road brings you to number 624 in Towaco. OpenStreetMap’s routing makes it a little under eleven miles, about 16 minutes without traffic.
Denville patients ask
Will you simply continue my current prescription? Not automatically. Your history is reviewed first, and your provider decides what to prescribe and at what dose.
What side effects are common? Mostly digestive ones such as nausea, diarrhea or constipation, usually strongest for a few days after a dose rises.
When can I come in? Monday to Saturday, with Tuesday appointments until 7 pm and Saturday ones until 1 pm.
Compare the medicines in semaglutide vs. tirzepatide, read about semaglutide side effects, or see the weight-loss injections page.

What Our Patients Say About Us
★★★★★
I have been to see Dr. D'Alessio over the past 5 years. I especially drive for over an hour, she and her whole office are simply the BEST! She has done my leg veins, Botox and now I see her for my GLP weight loss treatments, she is truly AMAZING and CARING! She will work with your insurance and help you find the best course of treatment to bring the cost down, if possible. She is always conservative, attentive, kind and has so much patience when listening to your exact needs. I would never go anywhere else. I trust her and her staff fully!
