When the scale stops listening
Weight that refuses to move is rarely a single problem. For some people it is appetite: hunger that comes back too soon and grows louder with every diet. For others, hormones have shifted, as with the weight that settles around the middle for many women near menopause, or the belly fat that men with low testosterone describe. Often it is a little of both. Montville MedSpa & Pain Center treats both sides in one practice, so you do not need to work out which it is before you walk in.
What the injections do
The two medicines prescribed here are each injected once a week. Semaglutide imitates GLP-1, a hormone the gut releases after eating; it acts on the parts of the brain that drive hunger and holds food in the stomach for longer, so a smaller meal satisfies. Tirzepatide acts on GLP-1 as well and adds a second receptor, GIP. Neither is prescribed until our medical team has reviewed your health, your goals and your history, and every plan pairs the medication with dietary recommendations, exercise guidance and regular check-ins.
What hormone therapy does
Hormone replacement therapy is a separate treatment with a separate purpose, and it is for men and women alike. It restores hormones the body no longer makes in sufficient amounts, testosterone for men and estrogen and progesterone for women, and it always begins with lab work. HRT does not by itself cause weight gain, and many patients find that balanced hormones make their weight easier to manage. Whether you need it at all is a question your own lab results help to answer.
How the two are weighed
- Your history first. What rules out a GLP-1 medicine (a personal or family history of medullary thyroid carcinoma or MEN2, or pregnancy) is not the same as what rules out hormones (hormone-sensitive cancers, blood clots, some heart and vascular conditions).
- Labs where they count. Hormone levels are measured before any hormone is prescribed.
- One practice, one set of notes. Whether one treatment, the other or both suits you is decided with your provider, and either is followed up in the same Towaco office.
Getting here from Riverdale
Drive south on Route 23 only as far as the Pompton Plains and Lincoln Park exit, take the Boulevard down through Pompton Plains, and turn right onto Jacksonville Road, staying on it until Main Road. That is about seven miles, or roughly 11 minutes without traffic.
Riverdale patients ask
Which side effects are most common with the injections? Digestive ones, mainly nausea, diarrhea, constipation or vomiting, usually at their worst in the days after a dose goes up.
Is the injection enough on its own? No. The medicine makes eating less easier to keep up; it does not replace the habits that keep weight off.
Which days are you open? Monday to Saturday, with Tuesday evenings until 7 and Saturdays until 1.
