What each visit is for
A weight-loss injection plan at Montville MedSpa & Pain Center is a run of appointments rather than a single prescription. Two medicines are offered: semaglutide, which copies the gut hormone GLP-1, and tirzepatide, which acts on GLP-1 and a second hormone receptor, GIP. Knowing what each visit asks of you, and what you take home from it, makes the first months far less of a mystery.
The first appointment: questions
Our medical professionals go through your health, your goals and your medical history before deciding which medicine suits you, if either. Expect to be asked about:
- type 2 diabetes, and any insulin or other medicine that lowers blood sugar
- past pancreatitis, gallbladder disease, kidney trouble or diabetic eye disease
- birth control pills, which tirzepatide can make less effective for a while after starting and after each increase
- pregnancy, now or planned, since neither medicine is used then
- any personal or family history of medullary thyroid carcinoma or MEN2, which rules both out
Bring a list of every medicine and supplement you take, any recent blood work, and an idea of your goal, whether that is a number, a marker your doctor flagged or simply feeling in charge of your appetite.
What you leave with
If one of the medicines is right for you, you go home with a plan in four parts: the medication and its dose, dietary recommendations, exercise guidance and a schedule of check-ins. The injection goes under the skin once a week, usually in the stomach, thigh or upper arm, on a day you keep the same. It starts low and is raised in steps, so early progress is often modest; that is by design.
What a check-in decides
Each check-in looks at how your appetite has changed, how you are tolerating the medicine and whether the dose, the eating advice or the exercise guidance should move. Digestive side effects, mainly nausea, diarrhea, constipation or vomiting, tend to be worst for a few days after a dose goes up. Mention them rather than skipping a dose, because the pace of increases can be slowed. Between visits, call promptly about severe stomach pain that will not settle, signs of dehydration, a lump or swelling in the neck, or swelling of the face or throat.
The long view
Both medicines are usually treated as long-term, because weight tends to return for many people once they stop. Staying on, adjusting or stopping is decided with your provider, and the habits built in the meantime, protein at meals, water, strength work and sleep, are what carry on afterwards.
From Lake Telemark
Green Pond Road runs south a little over four miles to Interstate 80. Head east, change to I-287 northbound, and at Exit 47 turn left along Main Road to 624 in Towaco. It comes to about 17 miles, some 24 minutes without traffic by OpenStreetMap’s routing; from near Hibernia it is closer to 20.
Rockaway Township patients ask
What if I miss a weekly dose? Follow the medicine’s own instructions or call the office, and never take two to catch up.
How much will I lose? Nobody can honestly promise a figure; it depends on where you start and how your body responds.
I have surgery coming up. Does it matter? Yes. Tell the surgical team you take one of these medicines, because slower stomach emptying matters for anesthesia.
